IF YOU HAVE ORIGINAL PACKING SLIP...
Just write your name, address and phone number and the reason for the return on the packing slip and enclose it in a box along with the merchandise you wish to
return and send it back to us. Customers are responsible for the ship-back costs. All requests will be processed within two to four business days upon receipt of your package in New York, NY.
IF YOU DON'T HAVE ORIGINAL PACKING SLIP...
Write down on a piece of :
Your order number (if you have it)
Your customer number (if you have it)
Your name and address (required)
IF EXCHANGING, PLEASE NOTE THE FOLLOWING:
- Items you would like to exchange
- Reason for exchange
- Any additional items you may want to purchase
(if purchasing additional items please enclose check or credit card information)
IF RETURNING, PLEASE NOTE THE REASON FOR RETURN:
Once you have this information filled out, box it up along with the item(s) to be returned.
You can ship it back using any carrier you'd like (FedEx, UPS of the United States Postal Service), but please note
that shipping costs are the customer's responsibility. Please send items back to:
